PAYMENT
Appointments can be secured with a credit or debit card. HSA, FSA, and HRA cards may be accepted—check with your provider. Payment is due at the start of your session. We accept cash, check, and cards (processing fees may apply). You may also choose to bill the card on file.
NO-SHOWS & CANCELLATIONS
Cancellations or no-shows with less than 24 hours’ notice will incur a 50% charge. Please contact us if illness or emergency prevents you from attending—fees may be waived.
SCHEDULING
We recommend booking multiple sessions in advance to ensure availability.
REFERRAL
No physician referral is needed to begin physical therapy at Awaken Wellness.
ACCESSIBILITY
ADA accommodations are available for those unable to navigate stairs. Please let us know when booking if you require assistance.
INSURANCE
Awaken Wellness Physical Therapy is an independent provider and does not participate in insurance contracts. This allows us to offer upfront pricing and full transparency without the limitations of insurance-driven care. Our cash-pay services are often more cost-effective, offering more personalized attention and potentially quicker recovery.
You may request an itemized bill for physical therapy services (including diagnosis codes, CPT codes, and costs) to submit to your insurance as an out-of-network claim. Reimbursement is determined by your insurance provider. Only physical therapy services qualify.
MEDICARE CLIENTS:
Medicare does not allow cash-pay physical therapy services. However, Medicare enrollees are welcome to book wellness or yoga sessions that may include massage, assisted stretching, and exercise for strength, flexibility, and balance.
